New Student Form & Agreement
If you’re a new student joining the studio for in‑person individual lessons, please complete the full New Student Packet, including the checklist. This packet is designed to give you a clear, helpful overview of how the studio works.
Whether you’re exploring lessons for the first time, gathering information, or already on the waitlist and wanting to be prepared, the packet covers everything you need to know — from studio policies (cancellations, makeups, pausing lessons, classroom expectations), to required materials, potential fees, available discounts, and all liability agreements.
It’s a simple, all‑in‑one guide to help you start your musical journey with confidence and clarity.
Scholarship Application
If your family qualifies as low‑income and you’d like to apply on behalf of your child, please download, complete, and submit this application along with the required essay. Be sure to follow all outlined guidelines to ensure your application is eligible for review.

Applicants must be between the ages of 12–17 to qualify.
Media Release Form
In some cases, especially for minors, the studio may request a Media Release Form (MRF). This form allows Eclectic Etude and Miss Meg to share select studio photos — such as recitals, group classes, or special events — on the website and social media for promotional and community‑building purposes.
Signing the MRF is completely optional, and all photos are used only as outlined in the agreement. Families retain their rights, and participation is always respected.
